Underlines are missing from last column on a report

On the layout tab of the report select Page Setup and change the left and right margins to .3 Inches or greater. Save and run the report again.
 
Different printer drivers have different tolerances. You may be able to decrease the size to be less than .3 but in most cases you cannot go below that setting.
